To protect the security of your record and maintain integrity of the CDR/Academy database, we are unable to change mailing addresses and email addresses based on a verbal requests. We encourage you to log into your MyCDR account by clicking here to update your mailing address and/or email address. 

You may also submit the completed Address Change Form to cdr@eatright.org or mail to: Academy of Nutrition and Dietetics or Commission on Dietetic Registration, 120 South Riverside Plaza, Suite 2190, Chicago, Illinois 60606.
